Forms

Before therapy begins, certain paperwork must be completed. This ethically and legally required process can take up much of the first session, using blank forms. However, it is also possible to review the paperwork together in only a few minutes, if you prefer to fill it out and send it in before arriving.

Every form can be finished entirely online. Online submissions are not encrypted (we receive them as email), but a printable version is also available so that you can mail your forms to us, if you prefer. A link to a printable version is provided in the instructions at the top of each form.

Required Forms

  • Intake

The Intake Form asks for your most thoughtful responses about you and your life. Each person who attends therapy should fill out their own complete form.

  • Informed Consent for Psychotherapy

The Informed Consent for Psychotherapy defines how our professional relationship begins, including the limits of confidentiality. Each person attending therapy is required to read and understand the informed consent form, and to raise any questions or concerns before signing it and sending it to us. Please also keep a copy for your records.

  • HIPAA

The HIPAA Notice of Privacy Practices is required by federal law. You may have seen similar forms when seeing your doctor. It describes how medical information about you may be used and disclosed, among other matters. There is nothing for you to fill out, but the signature of each person is required. Please also keep a copy for your records.

For Therapy From a Distance Only

  • Informed Consent for Telemedicine

The Informed Consent for Telemedicine adds to our standard Informed Consent the subjects essential for each person attending therapy who is not in the same location we are. As with the standard Informed Consent, each person attending therapy from a distance from the therapist is required to read and understand the informed consent for telemedicine form, and to raise any questions or concerns before signing it. In addition to the copy you give to us, please also keep a copy for your records. If you are not intending to attend therapy from a different location from ours, you do not need to fill out this form.

For EMDR Only

  • Informed Consent for EMDR

The Informed Consent for EMDR Form supplements the standard informed consent form, outlining requirements and agreements relevant to EMDR therapy. If you are not intending to use EMDR, you do not need to fill out this form. For more about EMDR, see our EMDR page.
